RESOLUTION REVOLUTION OFFICIAL RULES
1. NO PURCHASE NECESSARY TO ENTER OR WIN. PURCHASE DOES NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW.
2. ELIGIBILITY: Contest open to legal U.S. residents 18 years of age or older at time of entry, except employees of Cryo-Cell International, Inc. and their respective affiliates, subsidiaries, franchisees, suppliers, advertising and promotion agencies, judging organization and the immediate families or household members of each. Contest void where prohibited by law. Entrants agree that the Sponsor has the sole right to decide all disputes arising from this contest, including but not limited to eligibility, and that all decisions of the Sponsor are final and binding.
3. HOW TO ENTER: You may enter from 12:00 a.m. EST January 1, 2010 through 11:59 EST February 15, 2010. To enter the Contest, provide your contact information as requested on the Contest web page www.celle.com/resolution (name, email address, telephone number, mailing address). In addition, please provide a brief video of no more than 60 seconds length, describing what you do to take better care of yourself, and what stem cells mean to you. Your submission must be in one of the following file formats: .mp4, .avi, .mov, .wmv, .3gp or .flv. Your submission must be suitable for republication (e.g., it must not contain obscene or pornographic material, it must not contain defamatory statements and/or statements that may be considered offensive to individuals of a certain race, color, ethnicity, sexual orientation, religion or socioeconomic group). It also may not contain threats to any person, place or entity and must be an original statement (e.g., no copying copyrighted works). Limit one entry per person. Once submitted, the submission will not be returned. Sponsor is not responsible for lost, late, incomplete, mutilated or misdirected entries or video files that are corrupt, improperly formatted or otherwise cannot be viewed. All entries become the property of Cryo-Cell International, Inc. and will not be acknowledged or returned. By entering this Contest and uploading your submission you agree to these Official Rules and also agree that Florida courts have jurisdiction over any dispute or litigation arising out of these Rules. You further irrevocably grant to Sponsor and its agents the unconditional and perpetual right to post, display, publish, use, adapt, edits, and/or modify such submission in any way, in any and all media, for any purpose, without limitation, and without consideration to you. Sponsor reserves the right to disqualify any entries by persons determined by Sponsor to be tampering with or abusing any aspect of the Contest.
4. DRAWING: A winner will be chosen by judges appointed by Sponsor’s Marketing Department. The entries will be judged and ranked on the following criteria: creativity (30%), genuineness of entrant (30%) and overall appeal of entrant’s story (40%). Winner will be notified by phone and/or email on or about March 1, 2010 and must sign and return an affidavit of eligibility and a publicity/liability release (where permitted) within 21 days of notification (first attempted delivery). Sponsor's inability to reach a winner, the return of any prize notification as undeliverable and/or a winner's failure to timely respond to prize notification, sign and return the required affidavit/releases, or comply with any term or condition of these Official Rules may result in disqualification of winner, forfeiture of his or her interest in all prizes, and selection of a substitute winner from among all remaining eligible entries, using the Criteria described above. Winners are solely responsible for reporting and payment of any taxes on prizes. Prizes not won and claimed by eligible winners in accordance with these Official Rules will not be awarded and will remain the property of Sponsor.
5. PRIZE: One (1) Prize of Célle Menstrual Blood Stem Cell Enrollment, Processing and Testing, as well as free storage of the resulting specimen for 20 years will be given to the winner. There will be one (1) winner chosen. Approximate retail value: $2,479. No prize substitutions allowed and no cash alternatives, in whole or in part, are available, except that Sponsor reserves the right to substitute a prize of equal or greater value should a prize be unavailable at the time of prize award. Prize is non-transferable, except at the sole discretion of the Sponsor. By accepting prize, winner consents to the use of their name and/or likeness for advertising and publicity purposes worldwide without limitation and without additional compensation, except where prohibited by law. By accepting the prize, winner agrees to release, indemnify and hold harmless Sponsor and each of its employees, representatives, agents, representatives, advertisers, affiliates and subsidiaries from any and all liability, loss, damage, cost or claim relating to this contest. By accepting the prize, winner agrees to complete necessary Célle enrollment forms and schedules required by Cryo-Cell. Prize must be claimed and utilized by May 31, 2010. If for any reason the winner cannot utilize the prize by May 31, 2010, the Sponsor shall have no further obligation to the winner. All federal, state or other taxes on prize are the winner's sole responsibility.
6. ODDS OF WINNING: Odds of winning depend on number of entries received.
7. GENERAL RELEASE: By entering the Contest, contestants release Cryo-Cell International, Inc. and its respective affiliated companies, directors, officers, employees, representatives, partners and agents from any liability whatsoever for any claims, costs, injuries, losses or damages of any kind arising out of or in connection with the Contest or with the acceptance, possession or use of any prize (including, without limitation, claims, costs, injuries, losses or damages related to personal injuries, death, damage to, loss or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light).
8. WINNER'S LIST: For a list containing the prize winner's name, available after March 15, 2010, send a self-addressed, stamped (VT residents may omit return postage) envelope to Cryo-Cell Resolution Revolution, PO Box 919260, Orlando, FL 32891-9296 by March 31, 2010.
